Out of all the times I've been, I have yet to come across overflowing toilets.
Usually they are fairly clean and stocked. (relatively speaking - these are restrooms we are talking about, not restaurants...). Considering the volume they deal with, I think they do a pretty good job keeping them clean. Seen worse in fast food places outside of the park.

I suppose the restrooms near the RR crossing north of Gemini were clean enough for those 4 dudes who were in one stall when I was there last summer. There was a funny smelling cloud of smoke and 8 legs wafting from the stall, the kind of odor you may detect in Colorado this past year.
The staff does a very fine job of keeping them clean.
When it is chilly on Halloweekends the best restroom is the one along Frontier trail which is heated.
The worst are at old Cleveland Municipal Stadium in the 1970's where a man must stand in front of an overflowing trough and pray there are no holes in your converse all-stars. You might even find a lost female present.
They do a pretty good job keeping them clean, but I have NO IDEA how they smell so bad all the time, particularly considering they are open-air and don't have entry/exit doors. Lots of fresh air.
What I don't like, and you'll find this lots of places, is sinks that put out only a teeny tiny bit of water for you to wash your hands with. I hate it even more when I have to push a nasty button down, and then get only a few seconds of very low pressure, low flow water. Not enough to get the soap off, for sure.
Then again, the vast majority of men I've seen in the restrooms don't wash their hands anyway, so this probably doesn't bother them.
^If theres dog poop outside it still smells bad right, or cat litter box, etc.
Just because it's around alot of air doesn't mean it's getting ventilated well. Plus with ventilation that does spread the smell..It's good and bad at the same time. Don't want the restrooms to turn into a giant crop dusting machine do you?
