2004 passes...
I believe there is a form on their website that you fill out. I'm too lazy to look for it myself, but it's there somewhere.
Edit: I should clarify that it is a form you print and mail in with the passes. It has the mailing address on there.

They also sent you a renewal slip with your Season Pass certificates if you ordered them online or via mail. Just mail it all in and they'll send you your nice new pink pass.
