What to list on applications?

I'm filling out non-Cedar Point applications now and I'm trying to figure out what number to list for contact? I just moved and can't find any paperwork from last year that might tell me.

Also is there a specific name I should list?

I worked Halloweekends.

I always just put the main 0830 number.
