Our best chance of getting the real story is for S&S to issue a maintenance bulletin describing a vaguely similar sort of incident.
What about ride inspection reports from the state, since they are public record. What kind of information do they typically contain?
A State inspection report probably won't be terribly enlightening. It may include a checklist of inspection points on the ride, all of which passed. If the State inspector finds any deficiencies, those will be noted, along with an indication that those deficiencies were corrected or rendered non-hazardous.
Basically a State inspection would only be interesting if the ride fails, and the park isn't going to request reinspection unless they are pretty sure the ride is going to pass.
That's why a bulletin would be the most enlightening probability...from the manufacturer trying to prevent a reoccurrence on another machine.

Do they always issue those? Or do they sometimes just recommend changes to the park which are then made during the off-season?
I think back to the additional supports being added to Wicked Twister. That seems like a case where Intamin came to Cedar Point with design changes they recommended.
