Paper Work

I am going to be a new employee, and I was just curious on what is all the paper work you need to print and bring when you process in?

Your paperwork is available to print from the hrxtalx link that should have been sent to you in an email. There are some things online that you will have to fill out. There is also a pdf download that should include a medical history form, a waiver for Voluntary Participation, and a Food Borne Illness prevention sheet. These need to be completed and brought with you for when you arrive. There should also be a form W4 and Ohio IT4 for you to declare any tax withholdings. Lastly, you will need to bring your identity documents as shown on the I9 paperwork. You will need your social security card for payroll purposes and your housing deposit if you plan on living in the Cedar Point dorms.
