Parent Swap works like this: At the entrance of any attraction, request a parent swap from the host. He/she will ask for your last name and give you a pass. At this time, Split your party so that up to 2 people (at least one must be 14 or older) who wish to ride wait outside the queue with the child that does not meet the height requirement. The rest of your party may enter the line and wait. If you have fastlane, you may use that queue but everyone who intends to ride must have it.
Once you reach the platform, hand the pass to one of the hosts (on the exit side for most rides). They will retain your pass while you ride and then exit. At this time, meet up with your party and send up to 2 people who waited with the child up the exit while the people in your party who rode first stay with the child. After those 2 people have ridden, the ride hosts should give the pass back to you. If your pass is lost during the day, just obtain a new one from another ride entrance.

I have no need for this, but am asking just out of curiosity. Is there a size/age limit on the involved child? And if so, are there exceptions for a special needs individual who is not a "child"?

No requirement at all for parent swap, I have a 11 year old that is terrified of coasters.
There is a special needs pass for other situations, a friend has an autistic 16 year old that cannot stand in lines without it getting to be too much and gets over stimulated. They go to the ride entrance and get a time from the employee equal to the current wait time to come back and ride right away.
JTaylor is correct, There is a separate program for disabilities. You are getting into a grey area Rusty and it will boil down to the discretion of the ride hosts. I see no issue myself with issuing a parent swap for special needs or for a scared guest of a young age. If a person is not disabled and old enough to be left unsupervised and alone, then there might be potential for an issue. To my knowledge, there is not an established age/ height limit on the parent swap.
